CNY Family Care, LLP is seeking a highly skilled Nurse Practitioner/Physician Assistant to join our team in a hybrid position. As a Triage Nurse Practitioner/Physician Assistant, you will play a critical role in providing professional assessments, prioritizing treatments, and initiating medical care to patients of our practice.
Key Responsibilities- Respond to patient requests or medical advice questions from the patient portal in a timely and professional manner.
- Review and react to various patient documents, including but not limited to; surgeries, stress tests, CT scans, MRI results, Mammograms, Immunizations, etc.
- Update patient records in the Electronic Medical Record from review of documents.
- Communicate with patients' Primary Care Physicians with any significant change in health status.
- Current valid NYS Nurse Practitioner or Physician Assistant license.
- Minimum of two (2) years' experience working in a family practice or Urgent Care setting.
- Previous experience working with Medent (EMR) is preferred.
- Energetic and excellent working knowledge of chronic disease states.
- Critical thinking skills and ability to work independently.
