Key Responsibilities:
  • Assessment and Care Planning: Assess and develop personalized care plans for patients, focusing on the most suitable and least restrictive level of care.
  • Referral and Coordination: Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services.
  • Collaboration and Communication: Collaborate with patients, families, and healthcare providers to ensure comprehensive care.
  • Stakeholder Engagement: Communicate effectively with stakeholders to ensure quality coordinated care and services are provided efficiently.
  • Patient Advocacy: Partner with the care team to advocate for patients and families, ensuring their needs and choices are represented and supported.
  • Documentation: Document all member assessments, care plans, and referrals provided.
Working Schedule:

This is a field-based position with a home-based office, allowing for flexibility and work-life balance. The working schedule is Monday through Friday, 8-hour shifts between 8 am and 5 pm.

Location:

This position is based in Milwaukee or Green Bay and surrounding communities in Wisconsin.

Requirements:
  • Education: High School Diploma/GED or equivalent case management experience.
  • Experience: 1+ year of experience in field-based or case management settings.
  • Technical Skills: Intermediate level of computer proficiency, including Microsoft Outlook and Teams.
  • Transportation: Valid driver's license in Wisconsin and reliable transportation.
  • Soft Skills: Ability to work independently and maintain good judgment and accountability.
Preferred Qualifications:
  • Community Health Worker Accreditation: Community Health Worker (CHW) Accreditation.
  • Experience with Motivational Interviewing: Experience with motivational interviewing.
  • Knowledge of Medicaid/Medicare Population: Knowledge of Medicaid/Medicare population.
  • Behavioral Health Experience: Knowledge and/or experience with behavioral health or substance use disorders.
