Home Health Visits Licensed Practical Nurse

BAYADA Home Health Care is seeking a skilled Licensed Practical Nurse (LPN) to join our team as a Home Health Visits LPN. As a Home Health Visits LPN, you will be responsible for providing high-quality patient care to adult and geriatric clients in their homes.
Key Responsibilities- Develop and implement individualized care plans in accordance with patient needs and goals.
- Conduct home visits to assess patient conditions, provide medical care, and educate patients and families on self-care techniques.
- Administer medications, perform wound care, and provide other medical treatments as prescribed by physicians.
- Monitor patient progress, report changes to clinical managers, and make recommendations for adjustments to care plans.
- Collaborate with healthcare teams to ensure seamless care coordination and communication.
- Current licensure as an LPN in Massachusetts.
- Graduation from an accredited nursing program.
