About the Role

We are seeking a compassionate and skilled Licensed Clinical Social Worker to join our Palliative Care team at St. Mary's Hospital in Richmond, VA. As a key member of our interdisciplinary team, you will provide critical psychosocial counseling to patients and their families, helping them navigate the complexities of serious illness and end-of-life care.

Key Responsibilities
  • Provide emotional and psychological support to patients and families, addressing concerns related to illness, limited resources, and support.
  • Collaborate with the healthcare team to clarify culturally sensitive goals of care and treatment plans.
  • Assist patients and families in understanding, accepting, and forming realistic courses of action, maximizing their strengths and resources.
  • Assess bereavement risk and contact families post-death, referring to community resources as needed.
  • Participate in quality improvement initiatives, team efforts with other hospital departments, and outpatient and community services.
  • Collaborate with Care Management to coordinate team efforts with hospital support departments, ensuring smooth discharge planning and follow-up.
  • Develop and review protocols and processes supporting appropriate referrals to the Palliative Medicine team.
  • Participate in patient care quality and satisfaction measures.
  • Attend team meetings and participate in designated educational initiatives.
  • Assist patients with advance care planning, including discussions of illness, values, goals, quality of life, and other considerations.
  • Recognize and respond to ethical dilemmas, calling formal ethics consults as needed.
  • Assess risk factors for abuse, neglect, suicide, and mental health issues.
  • Utilize non-pharmacological approaches to symptom management, such as progressive relaxation and visualization.
Requirements
  • Bachelor's or Master's degree in Social Work.
  • LCSW licensure as required by state of practice.
  • Basic Life Support (BLS) certification.
  • 3-5 years of inpatient, outpatient, or home-based social work experience.
  • Experience in palliative care settings, such as hospice and/or other advanced illness settings.
  • High-level communication and organizational skills, with ability to work independently.
