Crisis Assessment Social Worker Full-Time Day Position

Job Summary:
We are seeking an experienced Crisis Assessment Social Worker to join our team. As a key member of our social work department, you will play a vital role in facilitating care along a continuum, ensuring that patients receive the support and resources they need to achieve optimal health outcomes. This position requires a deep understanding of complex psycho-social assessment and intervention, as well as excellent communication and interpersonal skills.
Responsibilities:
- Conduct thorough assessments of patients' needs, identifying potential risks and developing effective transition plans to promote timely throughput and safe discharge.
- Collaborate with multidisciplinary teams to coordinate care, communicate with patients and families, and provide education to support post-acute needs.
- Liaise with local, state, and federal agencies to address issues related to adoptions, abuse, and neglect.
- Document patient information accurately and maintain confidentiality at all times.
- Precept new staff members and serve as a resource to colleagues, promoting a culture of excellence and teamwork within the department.
- Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices that enhance patient care and outcomes.
Requirements:
- Masters of Social Work (MSW) degree from an accredited program.
- Minimum 2 years of acute hospital experience, preferably in a case management or crisis assessment role.
- Active licensure, certification, or registration to practice as a social worker, in accordance with state and federal regulations.
- Strong analytical and problem-solving skills, with ability to work effectively in fast-paced environments.
- Excellent communication, interpersonal, and leadership abilities, with capacity to build strong relationships with patients, families, and healthcare professionals.
Estimated Salary: $80,000 - $110,000 per annum, depending on experience and qualifications. Benefits include comprehensive health insurance, retirement plan, paid time off, and opportunities for professional growth and development.
