Job Summary:
Maxim Healthcare Services is seeking a skilled Licensed Practical Nurse (LPN) to provide high-quality patient care in a home health setting. As an LPN, you will be responsible for delivering patient-centered care, collaborating with healthcare teams, and adhering to industry standards and regulations.
Key Responsibilities:
- Assume responsibility for the application of the nursing process and the delivery of patient/client care.
- Provide services, as directed by contract and/or program, while under the supervision of a contract/program Registered Nurse or clinical designee.
- Utilize the nursing process to plan, implement, and evaluate patient/client care and/or services provided to client(s) receiving contracted services.
- Identify and document changes in patient/client physiological and psycho-social condition.
- Notify Director/Supervisor, and contracted customer designee if applicable, when physician notification is required in order to obtain necessary orders for intervention(s), per state regulations.
- Adhere to Maxim policy, procedure, and applicable protocols.
- Perform/provide interventions, treatments, and medication administration, according to identified priorities, the plan of care, facility policies, and patient/client care outcome standards as applicable.
Requirements:
- Graduate of an accredited practical nurse or vocational nursing program preferred.
- Currently licensed as a Licensed Practical Nurse license in specific state required.
- Complies with accepted professional standards and practice required.
- One year prior professional nursing experience preferred.
- Willingness to participate in skills advancement training or novice nurse program if applicable.
- Current CPR, as applicable, for direct patient care.
- PPD or chest x-ray if applicable.
- Successful completion of Maxim Orientation and Compliance Training, and all other assigned trainings as applicable required.
- Successful completion of Maxim's Competency Program required.
- Proficiency in the English language required.
- Current Health Certificate (per facility contract or state regulation) required.
- Must meet all federal, state, local, and contract requirements.
