We are seeking a positive Licensed Practical Nurse (LPN) to join our team as an Intake Coordinator.
In this role, you will have the opportunity to collaborate with our Office Manager and Director of Clinical Services, playing a vital part in our mission.
Our dedicated team will rely on you to ensure the office obtains all necessary documentation for patient admissions, including orders, histories and physicals, medication lists, and any other essential information.
Key Responsibilities:
- Support our site Monday through Friday, on-site at our Newton office, to assess appropriate levels of care and patient eligibility for hospice admission.
- Establish and maintain positive working relationships with current and potential referral sources.
- Assign referrals to each location and communicate schedules to the Director of Clinical Services promptly for admissions.
- Inform and consult with the Director of Clinical Services on all patient, personnel, and service management needs and challenges.
- Ensure confidentiality of all patient, physician, staff, and proprietary information.
- Gather, collate, and report referral statistics, including key customer referral trends.
- Conduct initial chart audits.
- Perform additional related duties as needed.

Qualifications:
- Are passionate about providing the best possible care to patients and their families.

- Have excellent verbal and written communication skills required, including telephone etiquette and negotiation skills.
- Have an associate degree or equivalent knowledge and experience, preferably in the hospice/healthcare field.
- Are proficient in Microsoft Office and maintain a positive and professional attitude.
