We are seeking a highly skilled and experienced Pharmacist to join our team at Mount Sinai Health System. As a Per Diem Pharmacist, you will play a critical role in supporting patient-focused care and ensuring the safe and effective use of medications.
Key Responsibilities- Dispense medications accurately and in a timely manner
- Resolve questions regarding formulary and non-formulary medications, adhering to policies and procedures
- Provide dosing and therapeutic recommendations based on patient age and kinetic variables
- Perform clinical chart reviews and patient care unit inspections
- Prepare parenteral medications using sterile aseptic technique
- Maintain adequate medication supply and inform supervisor of impending shortages
- Intervene and document interventions to correct or improve pharmacotherapy
- Report adverse drug events and medication errors
- Take responsibility for the operation of the assigned pharmacy area and activities of all supportive personnel
- 2 years of hospital pharmacist experience in Adult Critical Care or Pediatric Critical Care
- Ability to work every other weekend
- Graduate from an accredited College of Pharmacy (ACPE)
