The Position

This role involves conducting adult and geriatric assessments, medical history reviews, diagnoses, treatments, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.

You will work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as needed. Additionally, you will collaborate with Primary Care Physicians (PCPs) on patient education and provide follow-up support.

Responsibilities

  • Conduct thorough assessments and develop personalized plans for patients
  • Collaborate with healthcare professionals to ensure seamless care delivery
  • Provide education and support to patients and their families
  • Document all interactions and interventions accurately and efficiently

Requirements

To be successful in this role, you must have:

  • A Master's Degree in Nursing or equivalent experience with satisfactory completion of NP licensure
  • Current RN and NP licensure in the state of practice, including prescription authority or ability to obtain prescriptive authority
  • Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty
  • Current BLS, ACLS, or CPR certification
  • A minimum of 1 year of experience as a Nurse Practitioner
  • Strong computer skills and familiarity with Employee Health/Medical Record software
  • Excellent verbal and written communication skills
  • Ability to travel and adapt to changing situations

Compensation and Benefits

This position offers a competitive salary of $90.00 per completed assessment, mileage reimbursement, and opportunities for professional growth and development. As a non-benefited employee, you will also have access to a 401(k) plan and limited voluntary benefits.

Travel Requirements

This role requires the ability to travel to meet patients in their homes and other community environments. You must have a valid driver's license, proof of adequate automobile insurance coverage, and be comfortable with frequent travel.
