About the PositionThis position involves conducting adult and geriatric assessments, medical history reviews, diagnoses, treatments, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
You will work collaboratively with physicians, case managers, social workers, family members, key caregivers, and other ancillary medical personnel as needed. Your services will be provided in various venues, including home visits, skilled facility visits, and telehealth visits.
The ideal candidate will possess excellent verbal and written communication skills, strong computer skills, and familiarity with Employee Health/Medical Record software. You must also have a minimum of 1 year of experience as a Nurse Practitioner, current RN and NP licensure, Board certification, and BLS, ACLS, or CPR certification.
Benefits and RequirementsThis is a PRN/Part-Time opportunity with flexible scheduling. The hourly rate for this position is $90.00 per completed assessment. Mileage reimbursement and a 401(k) plan are also available.
The ability to travel, a valid state driver's license, and adequate automobile insurance coverage are required. As a Matrix Medical Network provider, you will be working in Gallup, New Mexico, and surrounding areas.
