Job Summary

We are seeking a highly skilled Physical Therapist to join our team at Center for Elders' Independence. As a Physical Therapist, you will play a vital role in providing restorative and rehabilitative services to our elderly participants.

Key Responsibilities
  • Collaborate with the interdisciplinary team to evaluate participants and develop individualized plans of care.
  • Provide physical therapy services to relieve pain, develop or restore function, and maintain maximum performance.
  • Conduct initial assessments and reassessments as needed, observing and recording participant responses to physical therapy.
  • Supervise and evaluate therapy program aides, providing in-service training as needed.
  • Attend interdisciplinary team conferences and participate in the development of participant plans of care.
  • Evaluate participants' home environments and make recommendations as needed.
  • Participate in coordination for 24-hour care delivery and delegate tasks to Physical Therapy Assistants as appropriate.
  • Document thoroughly and accurately, maintaining confidentiality and adhering to Physical Therapy practice standards and CEI policy.
Requirements
  • Possession of a minimum of a Bachelor's degree from a program of Physical Therapy approved by the Commission on Accreditation in Physical Therapy Education.
  • Current licensure as a Physical Therapist by the Physical Therapy Board of California.
  • Preferred, one (1) year of experience working as a Licensed Physical Therapist with the frail elderly population.
  • Current CPR and First Aid certification on file.
  • A valid California driver's license, current vehicle insurance, and access to a reliable vehicle.
