Job Summary

The Care Connections Nurse Practitioner plays a vital role in delivering high-quality, patient-centered care in the comfort of patients' homes, communities, and nursing facilities. As a key member of our team, you will provide comprehensive medical assessments, develop treatment plans, and coordinate care to ensure the best possible outcomes for our patients.

Key Responsibilities
  • Provide general medical care and coordination to patients of all ages, including adults, women's health, pediatric, and geriatric populations
  • Conduct comprehensive evaluations, including history and physical exams, to identify gaps in care and prevent complications
  • Address chronic and acute primary care complaints, and determine medical urgency
  • Develop and implement medical plans of care, including ordering diagnostic tests and consulting with specialists as needed
  • Provide post-discharge coordination to reduce hospital readmission rates and emergency room utilization
  • Participate in community-based 'Pop Up Clinics' to build relationships with community members and address health care gaps
  • Drive up to 120 miles per day to various locations within the assigned region, with occasional drives beyond 120 miles for Extended Mileage Special Project days
Requirements
  • Master's degree in family health from an accredited nursing program
  • Advanced computer skills, including proficiency with Word, Excel, and Electronic Medical Record systems
  • Active and unrestricted national certification from a recognized organization, such as the American Academy of Nurse Practitioners or American Nurses Credentialing Center
  • Current state-issued license to practice as a Family Nurse Practitioner
  • Current Basic Life Support for Healthcare Professional certification
  • Unrestricted driver's license
