About the Role

We are seeking a highly skilled Licensed Clinical Social Worker to join our Primary Care team. As a Patient Care Specialist, you will play a critical role in providing comprehensive care to our patients, working closely with our Primary Care Providers, Nurses, and other healthcare professionals to develop and implement individualized care plans.

Key Responsibilities

  • Conduct thorough assessments of patients' social and emotional needs, identifying areas for intervention and developing effective care plans.
  • Provide case management services to patients and their families, ensuring seamless transitions between care settings.
  • Lead group therapy sessions, focusing on topics such as PTSD, weight management, anger management, and coping skills.
  • Collaborate with patients and their families to develop healthy lifestyle choices and promote self-management of chronic conditions.
  • Maintain accurate and confidential patient records, adhering to all relevant laws and regulations.
  • Participate in ongoing education and training to stay current with best practices in patient care.

Requirements

  • Master's degree in Social Work from a program registered by the Department of Education.
  • Valid, unrestricted LCSW/MSW license in the state of employment or any U.S. state or territory, depending on VA contractual requirements.
  • Minimum 1-3 years' post-master direct counseling experience with adult patients.
  • Proven mastery of advanced sociological, psychological, and mental health care-related interventions and issues.
  • Certification in Basic Life Support and ACLS, as specified by individual VAMC contract.
  • Proficiency in written and spoken English.
  • Strong computer skills, including EMR experience.
