Job Title: Speech Language Pathologist - Home Health Care

Supplemental Health Care is seeking a skilled Speech Language Pathologist to join our team in providing high-quality home health care services in Des Moines, Iowa.

Job Summary:

We are looking for a dedicated Speech Language Pathologist to work with our partnering home health care provider in Iowa. The ideal candidate will have prior experience with OASIS and Medicare certified visits, as well as a strong commitment to delivering exceptional patient care.

Responsibilities:
  • Provide speech language pathology services to patients in their homes
  • Develop and implement individualized treatment plans to meet patient needs
  • Collaborate with healthcare teams to ensure comprehensive care
  • Document patient progress and outcomes
Requirements:
  • Graduate of an accredited SLP program
  • Licensed as a Speech Language Pathologist in Iowa
  • CPR certified or ability to obtain
  • Previous experience with computer charting
  • 6 months of previous home health care experience
