Southern Regional Medical Center is seeking a highly skilled and compassionate Patient Care Coordinator to join our team. As a key member of our healthcare team, you will be responsible for ensuring the quality and resource management of all patients admitted to our facility.
Key Responsibilities- Conduct comprehensive admission and concurrent reviews of medical records to ensure medical necessity, intensity of service, and severity of illness.
- Develop and implement individualized care plans to promote cost containment and achieve quality, clinical, and cost-effective outcomes.
- Collaborate with healthcare providers to integrate care and achieve seamless delivery of services.
- Provide education and support to patients and their families to promote self-management and empowerment.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Minimum 5 years of work experience post-graduation of an accredited school of nursing and a current state Registered Nurse license.
- Grandfathered prior to April 1, 2015, with a minimum of 5 years post-graduate experience from an accredited school of Social Work for Licensed Clinical Social Worker. However, RN Case Manager is preferred.
- Five years of acute care nursing experience is preferred, with at least one year of experience in case management, discharge planning, or nursing management.
- Current BCLS (AHA) certificate upon hire and maintain current.
- Knowledge of Milliman Criteria and InterQual Criteria is preferred.
- Experience and knowledge in basic to intermediate computer skills.
- Behavioral Violence Prevention Training within 3 months of hire and maintain current.
