**Job Summary:**

The University of Vermont Medical Center seeks a skilled Clinical Healthcare Specialist to join our team. As a key member of our healthcare team, you will play a vital role in ensuring the appropriateness of drug therapy for inpatients and outpatients.

**Responsibilities:**

* Collaborate with healthcare professionals to develop and implement effective medication plans
* Conduct thorough assessments of patients' medication needs
* Maintain accurate and up-to-date records of patient medication
* Provide education and support to patients and their families on medication management
* Participate in quality improvement initiatives to enhance patient care

**Requirements:**

* Bachelor of Science Degree from an ACPE accredited School of Pharmacy required
* Doctor of Pharmacy Degree preferred
* ASHP accredited Pharmacy Residency or equivalent experience preferred
* Licensed Pharmacist in the State of Vermont

**Preferred Qualifications:**

* Two years clinical pharmacist experience in a health system preferred
