Comprehensive Rehabilitation Therapist for Home Health Division

Tennessee Quality Care – an Addus family company is seeking a skilled Full-Time Physical Therapist or Physical Therapist Assistant to join their esteemed Home Health Division in Shelby County, TN.
About the Role
This critical position involves providing personalized physical therapy services to patients in the comfort of their own homes, assisting in evaluating functional levels, and developing treatment plans that ensure the highest quality of care. Key Responsibilities
- Deliver Patient-Centered Care: Provide comprehensive physical therapy services to patients according to a written physician's plan of care and as defined in the state Physical Therapy Practice Act.
- Evaluate Functional Levels: Assist in evaluating the level of function and develop treatment plans that address specific needs.
- Collaborate with Healthcare Team: Work closely with attending providers, nurses, and other healthcare professionals to ensure seamless patient care.
Requirements
- Credentials: A graduate of a School of Physical Therapy approved by the American Physical Therapy Association or Allied Health Education and Accreditation of the American Medical Association and the Physical Therapy Association, with current licensure as a Physical Therapist in the state of practice.
- Experience: At least two (2) years' experience in an acute hospital or equivalent experience.
- Physical Therapist Assistant Requirements: A graduate of an accredited physical therapy assistant program with either an Associate's degree in science or applied science that is approved by American Physical Therapy Association, Inc., with current licensure as a Physical Therapist Assistant in the state of practice, and at least one (1) year's experience as a Physical Therapy Assistant.
What We Offer
- Competitive Salary: $65,000 - $85,000 per annum, depending on experience.
