Job Summary

We are seeking a dedicated and compassionate LPN to join our team at Shared Support South, Inc. as a Home Care Specialist. In this role, you will provide high-quality nursing care to individuals with complex medical needs in their home and community settings.

Key Responsibilities
  • Ensure the well-being and safety of our consumers, providing personalized care in accordance with their individualized care plans and physician orders.
  • Provide home care nursing services to adult consumers with Trach, Vent, and G Tube dependencies, promoting a self-centered environment that focuses on their hopes, dreams, and goals.
  • Follow all care plans, ISP, and protocols, ensuring that our consumers' homes and medical equipment are clean and safe.
  • Lead in ensuring our consumers' health maintenance, including medication administration and G-Tube and J-Tube care.
Requirements
  • Current unencumbered nursing license (LPN) in the state of PA.
  • Current CPR certification.
  • Minimum 1 year LPN homecare experience.
  • Minimum 1 year Trach, Vent, and G Tube experience.
  • Graduate of an accredited nursing program.
Preferred Qualifications
  • Excellent documentation skills.
  • Proficient in G-Tube and J-Tube care and medication administration.
  • Ventilator experience.
  • Tracheotomy experience.
  • Compassionate, caring, and punctual.
