Job Summary

We are seeking a skilled Nurse Practitioner to be a core member of our multi-disciplinary Field Team, delivering high-quality, home- and community-based care to patients with complex needs.

Key Responsibilities
  • Partner with team members to develop and implement clinical treatment plans under the supervision of providers and registered nurses.
  • Support wrap-around clinical services in the home- and field setting, including co-visits with other team members and assisted telehealth visits with providers.
  • Provide clinical services and coordination as directed by a patient's clinical risk assessment and care plan.
  • Conduct health screenings for newly enrolled patients and on an ongoing basis.
  • Perform clinical services in the field, including wound care, phlebotomy, medication review, and patient status checks.
  • Meet with unable-to-reach patients in their home setting to re-engage them in care.
  • Provide cross-coverage for planned and unplanned absences to ensure patient care continuity.
  • Collaborate with the interdisciplinary team to improve care quality, reduce costs, and enhance patient and staff satisfaction.
Requirements
  • Licensed Practical Nurse, license certification in the state of employment.
  • Experience providing care for patients with complex needs, including mental illness and substance use disorders.
  • Current CPR and BLS certification.
