Job Summary:
The LPN Clinical Associate plays a vital role in assisting the Resident Care Manager in overseeing the Resident Care Aides, providing medication assistance, facilitating activities of daily living, and evaluating the medical and clinical needs of residents.
Key Responsibilities:
1. Monitor the physical, emotional, social, and cognitive well-being of residents.
2. Collaborate with the resident care coordinator to arrange necessary transfers to different care levels.
3. Keep physicians and families informed about changes in resident status and ensure adherence to physician care instructions.
4. Assist in assessing the functional capabilities of applicants as required.
5. Review all medical documentation with the resident care coordinator.
6. Ensure completion of admission documentation prior to resident entry.
7. Evaluate each resident's health and functional abilities.
8. Communicate health status updates to residents and their families.
9. Coordinate care needs and medical orders with residents' primary physicians.
10. Maintain accurate nursing records for all residents while ensuring confidentiality.
11. Direct and assign resident care in the absence of the resident care coordinator.
12. Ensure adequate staffing coverage at all times.
13. Assist in establishing medication assistance programs for residents as needed.
14. Supervise resident care assistants to uphold a safe medication administration process.
15. Maintain proper storage and documentation of medications not retained by residents.
16. Review and transcribe medication orders, ensuring compliance with health regulations.
17. Provide resident education regarding medications and medical equipment as necessary.
18. Participate in on-call duties as required to address resident and program emergencies.
19. Conduct training sessions for resident care assistants as needed.
20. Engage in quality assurance processes and conduct nursing program audits as necessary.
Qualifications:
- Valid nursing assessment skills.
- Strong collaborative approach to teamwork.
- Ability to interact courteously with staff, residents, families, and visitors.
- Current licensure as a licensed practical nurse.
- Minimum of three years of experience working with older adults.
- Registered nurse licensure may substitute for LPN requirements and some experience.
Physical Requirements:
The role requires occasional walking and standing, with the ability to lift up to 50 pounds without assistance. Specific vision abilities include close, distance, peripheral vision, and the ability to adjust focus.
Working Conditions:
Most essential duties are performed indoors and in resident areas, adhering to universal precautions. Employees may experience temperature fluctuations indoors.
Supervisory Relationship:
This position operates within established guidelines and requires periodic supervision, allowing for initiative and independent judgment.
Safety Responsibilities:
Employees must be familiar with all safety policies and procedures, actively participating in fire drills and emergency protocols as required. Confidentiality of all organizational and resident-related information is paramount.
