Job SummaryWe are seeking a Registered Nurse (RN) Case Manager to join our team. As a RN Case Manager, you will be the point of contact for all disciplines involved with providing care to patients and oversee the frequency of visits for the episode.
Responsibilities- Perform visits and complete coordination of client care
- Consult with the physician and office staff as needed to provide details about patient care
- Accurately collect OASIS data and ensure the medication profile remains current
- Report lab values to the physician in a timely manner
- Attend weekly case conference and monthly case manager meetings
- Currently licensed as a Registered Nurse (RN) in the state of employment
- A minimum of one year of clinical experience is preferred
- Demonstrate knowledge and skill in current nursing practice
- Possess a valid state driver's license and automobile liability insurance
- 30 Day PDO - Up to 6 weeks (including Holidays)
