Job Summary

We are seeking a Registered Nurse (RN) Case Manager to join our team. As a RN Case Manager, you will be the point of contact for all disciplines involved with providing care to patients and oversee the frequency of visits for the episode.

Responsibilities
  • Perform visits and complete coordination of client care
  • Consult with the physician and office staff as needed to provide details about patient care
  • Accurately collect OASIS data and ensure the medication profile remains current
  • Report lab values to the physician in a timely manner
  • Attend weekly case conference and monthly case manager meetings
Qualifications
  • Currently licensed as a Registered Nurse (RN) in the state of employment
  • A minimum of one year of clinical experience is preferred
  • Demonstrate knowledge and skill in current nursing practice
  • Possess a valid state driver's license and automobile liability insurance
