Job Summary

We are seeking a skilled Certified Wound Ostomy Nurse to join our team at Providence Home and Community Care. As a Wound Ostomy RN, you will provide high-quality care to patients with ostomies, draining wounds, fistulas, pressure ulcers, acute and/or chronic wounds, and incontinence in the comfort of their own homes.

Key Responsibilities
  • Coordinate and provide care to patients with complex wound and ostomy needs
  • Develop and implement individualized care plans to promote wound healing and ostomy management
  • Provide education to patients and their caregivers on wound and ostomy care, including proper wound dressing and ostomy appliance management
  • Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes
  • Perform related supply management and documentation tasks
Requirements
  • Bachelor's Degree in Nursing
  • California Registered Nurse License upon hire
  • National Provider BLS - American Heart Association within 30 days of hire
  • 1 year Nursing experience
Preferred Qualifications
  • 1 year CWOCN (ET) experience
  • 2 years RN experience
  • 1 year Home Health experience
