Key Responsibilities:
- Deliver exceptional patient care in a post-acute care setting, focusing on the unique needs of the elderly population.
- Manage new patient admissions and ensure smooth transitions and continuity of care.
- Collaborate with a multi-disciplinary care team to address and meet the comprehensive healthcare needs of senior living residents.
- Conduct patient visits, rounding, and complete documentation based on an agreed-upon schedule with facility partners and in response to day-to-day patient needs.
