Job SummaryAs a Licensed Practical Nurse (LPN), you will provide supportive nursing care to patients within an assigned unit under the supervision of a Professional Nurse. Your role involves performing various patient care activities and supportive services as delegated by the RN/Nurse Educator.
Duties & Responsibilities:- Interact with all individuals based on our ICARE values.
- Administer nursing care to residents as assigned.
- Assist in admission, daily care, discharge process, and participate in planning, implementing, and evaluating nursing care.
- Administer medications, treatments, and perform nursing procedures safely, competently, and timely.
- Document medical records per policy and procedure.
- Record care and services rendered accurately in a timely manner.
- Report resident changes in condition, pain, discomfort, concerns, and responses to RN/supervisor promptly.
- Reinforce, support, and provide healthcare instructions to residents and their families/healthcare representatives.
- Participate in team planning, implementation, and evaluation of nursing care and other resident activities.
- Observe, record, and report vital signs, symptoms, and general condition to appropriate personnel based on facility policy.
- Demonstrate safety awareness and follow protocols for fire, disaster, adverse events, infection control, and other safety practices.
- Maintain competency and proficiency in nursing care standards.
- Attend mandatory classes and participate in continuing education programs and other learning experiences.
- Complete all assigned education/in-services timely.
- Participate in performance improvement activities and committees as assigned.
- Foster an environment that promotes resident well-being, including abuse/neglect prevention, person-centered care planning, and delivery.
- Report any knowledge of abuse, neglect, mistreatment, misappropriation of property, and exploitation immediately based on policy.
- Assist in training new LPNs as assigned by RN/Nurse Educator.
- Perform other nursing functions related to resident care and nursing service as delegated by Nursing Service Administration.
Must be currently licensed or authorized to practice as a LPN in New York State. A minimum of one year of long term care experience is strongly preferred.
