About the Role

As a Nurse Practitioner, you will be responsible for managing patient care, conducting assessments, and developing treatment plans in collaboration with other healthcare professionals. You will also participate in quality improvement initiatives and contribute to the development of policies and procedures.

Key Responsibilities:

  1. Co-management with Primary Care Provider (PCP):
  • Provide direct primary care to patients as if you are the PCP, working independently to manage both chronic diseases as well as acute illnesses when PCP is unavailable
  • Routine-chronic disease continuity, pre-op eval or employment physicals
  • Urgent evaluation of acute complaints
  • Evaluate up to 5 revisit patients per session
  • Review all ordered laboratory, radiographic and ancillary studies
  • Document medical evaluation in the Epic Electronic Health Record (EHR)
  • Complete medication refills, patient forms and laboratory re-orders
  • Participate in inter-disciplinary care management of high-risk patients
Emergency Department (ED) Discharge / Inpatient Discharge Visits:
  • Perform focused histories and physicals on patients referred to the PCSN from the ED or from a recent inpatient admission if they are otherwise unable to get an appointment with their usual PCP
  • Order appropriate laboratory, radiographic and ancillary studies, as indicated
  • Make referrals to specialists, as indicated, and coordinate future care with the PCP
  • Document evaluations in Epic EHR
Triage Between Visit Care:
  • Help coordinators to triage appointment requests (i.e. urgency, correct provider, waitlist help)
  • Participate in GIM quality improvement activities
  • Review charts/reports for collaborative care and population health management

Requirements:

  • A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED)
  • Current certificate as a Nurse Practitioner issued by the NYSED
  • Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA)
