About the Role

We are seeking a highly skilled and compassionate Mobile Community Support Person to join our team at Advocates. As a key member of our care coordination team, you will play a vital role in providing accessible, holistic, and comprehensive care for individuals supported by our organization.

Key Responsibilities
  • Collaborate with individuals receiving services, medical teams, and behavioral health providers to provide integrated care management services within the community.
  • Coordinate services with existing providers and other stakeholders to ensure seamless care transitions.
  • Participate in team meetings to ensure effective communication among all disciplines involved in patient care.
  • Monitor patient progress and assist the clinical team with evaluating the need for continued services.
  • Identify community resources and develop natural supports for patients.
Requirements
  • BSW degree or BA in a related field from an accredited college/university or 1+ years of related experience.
  • One + years of direct care and/or care coordination experience or related experience.
  • Ability to use an Electronic Health Record to document medically necessary clinical services.
  • Must be able to perform each essential duty satisfactorily.
  • Ability to communicate effectively verbally and in writing.
  • Bilingual/trilingual (Spanish/Portuguese) preferred; candidate will have a demonstrated understanding of and competence in serving culturally diverse populations.

  • Must have a valid driver's license and access to an automobile.
