We are seeking a skilled Physical Therapy Assistant to join our team in a sub-acute/skilled nursing facility setting. As a Physical Therapy Assistant, you will work collaboratively with other rehab therapists and nursing staff to provide comprehensive and compassionate therapy services to our residents.
Responsibilities- Provide physical therapy services to residents in a sub-acute/skilled nursing facility setting
- Work collaboratively with other rehab therapists and nursing staff to optimize patient outcomes and patient discharge planning
- Utilize electronic documentation and Microsoft Outlook, Word, Excel to maintain accurate records and communicate with team members
- Participate in interdisciplinary team meetings to discuss patient care and progress
- Current Physical Therapy Assistant license or license eligible to practice in the state employed
- Strong computer skills with Microsoft Outlook, Word, Excel and experience with electronic documentation
- Good attention to detail and timeliness with documentation
- Ability to work well with others in a team setting
