Job Summary

We are seeking a skilled Case Manager to join our Refugee Health and Wellness Program. The successful candidate will provide direct case management services to trauma-affected refugees and immigrants, utilizing a trauma-informed strengths-based approach.

Key Responsibilities
  • Conduct person-centered, culturally-considered, and trauma-informed wellness assessments with program participants
  • Identify client's risk and protective factors, including their strengths, needs, abilities, and preferences, through holistic assessment
  • Respond to immediate mental health crises, provide stabilization, and connect individuals with emergency services if needed
  • Work with program Social Worker and clients to develop personalized care plans to ensure access to medical, mental health, and social service programs
  • Maintain regular contact with clients, including regular home visits, to monitor progress and address emerging needs
  • Identify ongoing client needs and coordinate with program Social Worker to ensure receipt of available services and resources
  • Advocate on behalf of clients to ensure access to appropriate resources, benefits, and services
  • Coordinate and/or provide transportation to and from program activities, appointments, and other essential services
  • Collaborate with agency staff and external care providers to ensure seamless service delivery and care coordination
  • Uphold clients' rights, privacy, and dignity and adhere to all contractual regulations
Requirements
  • Minimum of a High School Diploma or GED, bachelor's degree in social work or a related field preferred
  • Skilled in interpersonal and intercultural communication
  • Ability to be self-directed with position responsibilities
  • Strong organizational skills
  • Advanced skill with Microsoft Office and Google Apps including Google Drive, e-mail, and internet
  • Possess professional/academic abilities in English across all skill areas including clear pronunciation in speaking and correct grammar and spelling in writing
  • Experience facilitating partnerships with community medical and social service providers
  • Participate in a required background check
  • Valid driver's license, reliable vehicle, and current automobile insurance with clean driving record and willingness to drive clients
  • Experience working with diverse populations and limited English-speaking communities
  • Experience working with an interpreter preferred, but not required
  • Fluency in a language spoken by the communities served is strongly preferred, but not required
