Case Management Extender

About the Role:

We are seeking a highly motivated and compassionate Case Management Extender to join our team. As a Case Management Extender, you will play a critical role in navigating and accessing community services and resources, providing support through population health programs and care coordination activities.

Key Responsibilities:

  • Engage and motivate members to participate in case management programs by clearly articulating goals, benefits, and interventions.
  • Assist with the development of family-centered care and collect information for tailored assessments regarding case management eligibility.
  • Facilitate communication and collaboration amongst the healthcare team and provide personalized navigation support to members.
  • Connect families to resources to address social determinants of health and accommodate the specific cultural and linguistic needs of all patients.
  • Manage and monitor transitions between settings, caregivers, and providers, providing follow-up across the continuum of care.
  • Perform outreach to PCP/POC's, specialists, and home care providers to research and facilitate referral for services.
  • Develop patients and family's self-management skills through education and resource provision.
  • Manage administrative functions to support program and prepare and maintain records and case files.
