Part Time Physical Therapist

We are seeking a skilled Physical Therapist to join our team at Northwest Healthcare Center. As a Physical Therapist with CommuniCare Family of Companies, you will have the opportunity to make a direct impact on the lives of our residents through therapy.
Key Responsibilities:
- Evaluate and develop treatment plans for patients/residents in accordance with their needs.
- Implement treatment plans through direct treatment and education of the plan of care to team members.
- Attend and lead interdisciplinary meetings to relay clinical findings and make recommendations.

Qualifications:
