- Bachelor's Degree in Physical Therapy from an accredited program.
- Current Washington Physical Therapist License.
- National Provider BLS - American Heart Association certification.
- Specialized physical therapy training or experience may be required.
- Master's Degree in Physical Therapy from an accredited program.
- Ph.D. in Physical Therapy from an accredited program.
