Social Worker Assistant BSW Home Health Professional

We are seeking a skilled Social Work Assistant to join our team at Amedisys. As a Social Work Assistant, you will work under the supervision of a qualified social worker to provide social work services to patients and families in a home health setting.
Key Responsibilities- Provide social work services to patients and families in accordance with the physician's plan of care.
- Assist and counsel patients and families with health-related financial, social, and emotional concerns.
- Perform accurate and comprehensive assessments of patients' needs.
- Communicate changes in patients' clinical status to other healthcare team members and care center office staff in a timely manner.
- Coordinate care planning and delivery with the physician, patient/family, and other healthcare team members to facilitate optimal patient outcomes.
- Provide medical social work interventions and treatment based on ongoing patient assessments/reassessments and in accordance with physician orders.
- Educate patients and family members on medical social work interventions and treatment plans.
- Participate in interdisciplinary team conferences/patient care conferences in accordance with care center policy.
- Develop, prepare, and maintain individualized patient care progress records with accuracy, timeliness, and according to policies.
- Baccalaureate degree in social work, psychology, sociology, or other field related to social work.
- At least 1 year of social work experience in a healthcare setting or 2 years of experience as a social work assistant.
- Ability to work under the supervision of a qualified social worker.
