Nurse Practitioner

We are seeking a skilled Registered Nurse to join our team at Northeast GA Home Health, a part of LHC Group. As a Registered Nurse, you will have the opportunity to provide high-quality patient care in a home setting, working closely with our interdisciplinary team to deliver comprehensive care to our patients.
Responsibilities
As a Registered Nurse, you will be responsible for:
- Conducting initial and comprehensive nursing evaluations to determine primary focus of care and develop individualized plans of care.
- Providing and directing provisions of nursing care to patients in their homes, as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies.
- Coordinating total plan of care with other healthcare professionals involved in care and helping to achieve and maintain continuity of patient care.
- Ensuring patients meet home health eligibility and medical necessity guidelines as defined by payer source.
Requirements
To be successful in this role, you will need:
- A current RN licensure in the state of practice.
- A current CPR certification.
- A current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
