About the Role

We are seeking a compassionate and skilled Licensed Practical Nurse II to join our team at the Department of Developmental Services. As a LPN II, you will play a vital role in providing exceptional nursing care and support to individuals with developmental disabilities in community-based settings.

Key Responsibilities
  • Implement individualized health care plans, including vital sign monitoring, wound care, and medication administration.
  • Assist with daily living activities, such as personal care and maintenance, as outlined in the Individual Service Plan (ISP).
  • Initiate referrals for medical services, including scheduling, hospitalization, and follow-up care.
  • Communicate health issues to staff and update families/guardians on hospitalizations, rehabilitation, and other medical matters.
  • Order medications and maintain medical supplies, materials, and equipment.
  • Provide training to direct support staff on equipment use and treatments.
  • Transport individuals to work, medical appointments, and community engagements as needed.
Requirements
  • Current, active license to practice as an LPN in the Commonwealth of Massachusetts.
  • Valid Class D Motor Vehicle Operator's License.
  • Health Care Provider Basic Life Support (BLS) certification.
Preferred Qualifications
  • Demonstrated ability to develop and maintain respectful, caring relationships with individuals with intellectual and developmental disabilities.
  • Capacity to assist individuals with physical disabilities and/or limited mobility.
  • Knowledge of standard nursing policies, procedures, methods, and techniques.
  • Capacity to maintain accurate records and exercise discretion in handling confidential information.
  • Familiarity with safety practices and medical terminology/abbreviations.
