Job Description

Interfaith Community Services is seeking a highly motivated and compassionate Substance Abuse Outreach Counselor to join our team. As a key member of our outreach team, you will work closely with individuals experiencing homelessness, providing mobile outreach and support services to help them overcome Substance Use Disorder (SUD) and co-occurring mental health needs.

Key Responsibilities:

  • Conduct outreach and engagement activities to identify eligible individuals and households
  • Screen and assess referrals from local law enforcement and social services agencies
  • Collaborate with multidisciplinary teams to coordinate client care
  • Attend outreach and care conferencing meetings
  • Facilitate continuum of care and system navigation for clients

Requirements:

  • High School Diploma or equivalent required
  • Current certification or registration in substance abuse counseling
  • 2+ years of experience working in addiction treatment or social services
