Option Care Health is seeking a skilled Clinical Transition Liaison to join our team in Dallas, TX. As a key member of our organization, you will play a critical role in ensuring seamless transitions for patients from hospital settings to home or alternative sites of care.

Salary: $73,916 - $123,186 annually

Job Description
  • Maintain and grow relationships with referral sources to increase sales and patient starts.
  • Interact with area service providers daily to sell Option Care services that can assist discharging patients.
  • Conduct assessments of selected patients to ensure viability for alternative site care.

Key Responsibilities:

  1. Provide education, assistance, and service to referral sources as the primary point of contact.
  2. Utilize sales techniques to educate patients, families, and facility staff about services and products provided by Option Care.
  3. Effectively communicate with agency staff, medical teams, patients, and family throughout the discharge process.

Requirements:

  • Licensed Registered Nurse (RN) or licensed Pharmacist in the state of practice with at least 2 years of experience in the healthcare industry.
  • Basic PC skills, including internet, email, Microsoft Office applications.
  • Able to plan, organize, and make presentations.
