POSITION SUMMARY
Registry Ally is redefining the care delivery model for individuals facing Serious Mental Illness (SMI). The Physician Assistant will spearhead the medical components of our team-based care approach for individuals with serious mental health challenges. This role entails thorough documentation and support of the medical needs of clients through comprehensive clinical assessments aimed at identifying and addressing care gaps. The Physician Assistant will also facilitate care coordination among various medical and behavioral health providers, making referrals as necessary, and providing clinical guidance to peer support staff and social workers.
KEY RESPONSIBILITIES
Conduct comprehensive clinical assessments, including:
- Collecting information from external sources, particularly other healthcare providers
- Evaluating both behavioral and physical health through detailed history and examination
- Identifying undiagnosed conditions, including ordering additional tests as required
- Organizing information to support disability applications when appropriate
- Conducting home visits or field assessments as necessary
- Making tailored recommendations and referrals for programs and services based on assessed needs
Lead the medical aspect of our collaborative care model, including:
- Training and advising peer support staff on clinical matters
- Assisting the care coordination team
- Overseeing care management processes
- Identifying actionable care gaps
- Performing medication reconciliation post-discharge and as needed
- Communicating effectively with clients' healthcare providers, particularly at community mental health centers and hospitals
QUALIFICATIONS
The ideal candidate will possess:

- Intermediate computer skills, including proficiency in Microsoft Office Suite
- A valid driver's license and private auto insurance
EXPERIENCE
Successful candidates will have:
- A minimum of 5 years of clinical experience as a Physician Assistant, preferably in a primary care setting (must be licensed in the relevant state)
- Experience working with underserved populations facing behavioral health and substance use challenges
- Experience in developing and delivering educational programs for healthcare staff
- Proficiency in care management and coordination
- HCC Coding experience is preferred but not mandatory
