Clinical Liaison

At Inbound Health, we are seeking a highly skilled Clinical Liaison to serve as the face of our Hospital-at-Home program. This individual will be responsible for coordinating patient care activities, working collaboratively with internal and external partners, and promoting the program to cultivate growth.
Key Responsibilities- Drive Growth Objectives
- Develop an outreach strategy to increase utilization
- Create and implement plans to achieve business goals
- Identify new opportunities to increase patient enrollment
- Develop Clinical Recommendations
- Review and assess patient eligibility for the Hospital-at-Home program
- Make recommendations to discharge planning teams and hospitalists for qualifying patients
- Discharge Planning
- Assess prospective patients for eligibility and facilitate transition from acute care settings
- Prepare patients for discharge by assisting with orders and gathering clinical information
- UM Function
- Perform Utilization Management functions on behalf of customers, if needed
- Education
- Cultivate and maintain relationships with target sites to educate on program eligibility criteria
- Collaborate with site personnel regarding referrals, processes, and resources
- Documentation
- Complete required paperwork, manage correspondence, and maintain data in database for continuous improvement opportunities
- Bachelor's or Master's Degree in Nursing
- 2+ years of nursing/direct care experience in a healthcare setting
- Registered Nurse
- Experience in marketing/program coordination/program management/public health/home health
- Knowledge of Stark Laws
- Strong computer skills, including SharePoint, Excel, and EMR
- Strong verbal and written communication and interpersonal skills
- Excellent organizational skills
- Active clinical license
- Valid Driver's License
