About the Role:

  • Develop long-term relationships with patients and their families, providing high-tech nursing care to technology-dependent individuals in their homes.
  • Collaborate with the healthcare team to ensure comprehensive patient care, performing assessments, administering medications, and educating patients and families.
  • Utilize innovative technology to monitor and manage complex medical conditions, ensuring the best possible outcomes for our patients.

Requirements:

  • A current LPN license in Vermont.
  • Two or more years of directly related home/public health, hospice, and/or maternal/child health nursing experience.
  • Previous home care experience is a plus, but not required.
