We are seeking a skilled and compassionate Registered Nurse Case Manager to join our team at Adoration Home Health and Hospice. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality care to our patients and their families.
Responsibilities- Conduct comprehensive assessments of patients and families to identify their unique needs and develop personalized care plans.
- Collaborate with the healthcare team to ensure seamless care coordination and effective communication.
- Administer medication and treatment as prescribed by the physician, and provide education to patients and families on their care and treatment.
- Provide emotional support and counseling to patients and families, and facilitate their involvement in the care planning process.
- Maintain accurate and detailed clinical notes, and participate in interdisciplinary team meetings to ensure comprehensive care.
- Ensure compliance with all state and federal regulations, and adhere to Adoration's policies and procedures.
- Graduate of an accredited nursing school with current licensure to practice in the state of operation.
- At least one year of nursing experience, with three or more years preferred.
- Experience in hospice or a similar setting is highly desirable.
- Knowledge of the hospice philosophy of care and commitment to clinical and documentation excellence.
