Job Summary

Pathwell Health is seeking a skilled Physical Therapist Assistant to join our team in providing exceptional care to patients in their homes. As a Physical Therapist Assistant, you will work under the supervision of a licensed Physical Therapist to provide ongoing therapy services and contribute to the review of treatment plans.

Key Responsibilities
  • Provide therapeutic exercises as planned, delegated, and supervised by the Physical Therapist
  • Effectively communicate with the patient's care team and follow instructions given by the assigned Physical Therapist
  • Perform ongoing therapy services, including application of heat and cold, massage, gait training, and range of motion exercises
  • Accurately document care and patient progress, meeting established deadlines and regulatory requirements
  • Attend meetings, such as team meetings and in-services, to stay up-to-date on best practices and agency policies
Requirements
  • Graduate of a Physical Therapy Assistant Program, accredited by the American Physical Therapy Association
  • Current and valid Connecticut Physical Therapist Assistant license
  • Recent work experience in home health (preferred)
  • Driver's License, Car Insurance, and Car Registration
  • Reliable transportation and ability to drive in all types of weather conditions
  • Ability and willingness to follow agency policies and procedures and work as a team member
