About the Role

We are seeking a skilled and compassionate RN to join our team as a Hospice Clinical Liaison. This role is critical in building relationships with healthcare professionals and identifying new referral sources to increase hospice services' census and preference.

As a Hospice Clinical Liaison, you will utilize your clinical expertise and marketing skills to educate and guide prospective patients, families, and referral sources about hospice services. You will be responsible for outreach and relationship building, promotion and positioning, brand building, referral generation, patient eligibility, and patient and family meetings.

Key Responsibilities
  • Develop and maintain strong relationships with medical community members and partners to identify new referral sources and increase hospice services' census and preference.
  • Promote quality care and unique capabilities to medical facilities and healthcare professionals.
  • Establish the hospice as the preferred provider in our service area, striving to exceed growth and census goals.
  • Generate new referrals through planned calls to physicians, facilities, case managers, discharge planners, and social workers.
  • Assist in determining patient eligibility based on hospice knowledge and training.
  • Conduct initial meetings to provide an overview of hospice services and obtain consents for care.
Qualifications
  • Education: Bachelor's degree in nursing or healthcare preferred.
  • Licensure: Current unencumbered license as a registered nurse.
  • Skills: Strong relationship-building skills with key staff, marketing contacts, and clinical staff. Proven ability to generate and maintain relationships with new and existing accounts, including physicians, healthcare professionals, and community agencies.
