About the Role

We are seeking a skilled and compassionate Nurse Practitioner to join our team in providing high-quality care to patients in long-term care facilities.

Responsibilities
  • Provide comprehensive healthcare services and coordinate patient care
  • Work closely with a team of healthcare professionals to ensure seamless care
  • Assess, diagnose, and treat residents' physical and mental well-being
  • Conduct thorough patient assessments, including medical history reviews and physical examinations
  • Order and interpret diagnostic tests
  • Develop accurate diagnoses and treatment plans
  • Prescribe medications and administer treatments
  • Provide counseling to residents and their families
  • Collaborate with the interdisciplinary team for coordination of care
  • Manage chronic conditions and monitor progress
  • Make adjustments to care plans as needed
Requirements
  • Nurse Practitioner License
  • DEA License
  • 6 months experience in long-term care
