Job Title: Weekend Registered Nurse RN Home Health

Elara Caring is seeking a compassionate and dedicated Registered Nurse Home Health to join our team. As a Registered Nurse Home Health, you will provide high-quality care to patients in their preferred home setting, working autonomously and collaboratively with our team to deliver unparalleled care.

Key Responsibilities:
  • Assess and plan patient care, including developing and implementing individualized care plans.
  • Provide direct patient care, including administering medications, performing wound care, and providing emotional support.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
  • Document patient progress and outcomes, ensuring accurate and timely reporting.
  • Participate in ongoing education and training to enhance clinical skills and knowledge.
Requirements:
  • Current, unrestricted state RN license.
  • 1+ year experience in a clinical care setting as a nurse.
  • Able and willing to travel within branch/office coverage area.
  • Must have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws.
  • Must be able and willing to travel 50%.
  • Associates Degree or Bachelor's Degree in Nursing is preferred.
  • Previous Home Health or Hospice Experience is preferred.
  • Previous experience with HomeCare HomeBase is preferred.
