Job Summary

We are seeking a highly skilled Licensed Practical Nurse to join our team at UW Health Rehabilitation Hospital. As a Licensed Practical Nurse, you will play a vital role in providing exceptional patient care and contributing to the success of our rehabilitation programs.

Key Responsibilities
  • Assess and reassess patients to identify their individual needs and develop effective care plans.
  • Administer medications and treatments as prescribed by physicians and registered nurses.
  • Collaborate with interdisciplinary teams to ensure seamless patient care and achieve optimal outcomes.
  • Provide education and support to patients and their families to promote independence and self-care.
  • Participate in quality improvement initiatives to enhance patient satisfaction and outcomes.
Requirements
  • Graduate of an accredited licensed practical/vocational nurse program.
  • Current state licensure as a Licensed Practical/Vocational Nurse.
  • CPR/BCLS certification. ACLS preferred.
  • IV Certified required or obtained within 6 months highly preferred.
  • Minimum six months' Medical/Surgical experience in an acute care setting preferred.
  • Excellent oral and written communication and interpersonal skills.
