Job Summary

We are seeking a Medical Social Worker to join our team at NHC HomeCare Anderson. The successful candidate will supervise and provide evaluations of patients' social and emotional needs related to their medical condition, providing counseling and support based on this evaluation. This role requires a Master's Degree in Social Work and one year of experience in a healthcare setting. In addition, specific state requirements apply, such as licensure by the South Carolina Board of Social Work Examiners at the Master (LMSW) or Independent (LISW) level.

Key Responsibilities

* Assess social and emotional factors to minimize impact on patients' health and ability to cope with daily living challenges* Help patients and caregivers understand, accept, and follow medical recommendations* Assist patients and caregivers with personal and environmental difficulties that predispose towards illness or interfere with obtaining maximum benefits from medical care* Identify resources, such as caregivers and community agencies, to assist patients in resuming life in the community, including discharge planning* Collaborate with providers and home care team members to understand significant social and emotional factors related to health problems* Participate in the development of the plan of care

Benefits and Compensation

This position offers a competitive salary, estimated at $55,000 - $65,000 per annum, depending on qualifications and experience.
