Key Responsibilities
  • Collaborate with physicians and the interprofessional healthcare team to provide patient-centered care and achieve desired treatment outcomes.
  • Conduct thorough assessments and develop comprehensive psychosocial treatment plans to address patients' emotional, social, and environmental needs.
  • Facilitate discharge planning activities, including identifying community resources and coordinating care transitions.
  • Provide education and support to patients and families on health-related issues and connect them with community resources.
  • Participate in quality improvement initiatives and contribute to the development of evidence-based practice projects.
Requirements
  • Master's degree in Social Work from an accredited program.
  • Licensed Medical Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) in the State of Texas.
  • 0 years of experience, with new graduates and those with hospital internship experience preferred.
