Physical Therapist

We are seeking a skilled Physical Therapist to join our team at Visiting Nurse Association of Ohio. As a Physical Therapist, you will play a vital role in providing home care therapy to individuals, helping them restore function, improve mobility, and relieve pain.
Responsibilities:- Conduct initial assessments to determine patient treatment needs and develop personalized plans of care.
- Provide direct physical therapy services, including performance of physical therapy procedures, instruction of patients and caregivers, and recommendations for equipment or adaptation of home environments.
- Re-evaluate and revise plans of care as needed with physician approval.
- Collaborate with other disciplines to facilitate total care services for patients.
- Ensure timely submission of all documents and obtain physician approval for modifications to treatment plans.
- Bachelor's degree in Physical Therapy preferred.
- One year of experience in a supervised healthcare setting.
- Ability to work effectively with diverse populations and collaborate in a complex institutional structure.
- Strong planning and organizational skills, with the ability to work well under pressure and maintain attention to detail.
- Ability to work independently with minimal supervision and maintain confidentiality.
