Job Description
We are seeking an experienced Licensed Social Worker to join our hospice team as a Medical Social Worker. In this role, you will work closely with our interdisciplinary team to provide medical social services to patients and their families, ensuring that their physical, emotional, and social needs are met. Your primary responsibilities will include:
    Assisting patients and their families in understanding the significance of personal, emotional, environmental, and social factors related to their health status.
    Contributing to the development of a comprehensive plan of care for patients, in collaboration with the healthcare team.
    Instructing healthcare team members on community resources available to assist patients, as needed.
    Providing bereavement services and supervising the provision of these services, reflecting the needs of patient families.
    Establishing a plan of care that addresses bereavement needs, with clear delineation of services to be provided and frequency of service delivery, up to thirteen months following the death of the patient.

Salary
The estimated annual salary for this position is $65,000-$85,000, based on industry standards and your level of experience.

Required Skills and Qualifications
To be successful in this role, you should have a Bachelor's Degree in Social Work or a related field, with at least one year of experience in a healthcare setting. You must also be licensed as a Social Worker from the State Board of Social Work in Arkansas.
