As a Healthcare Navigator with Arkansas Hospice, Inc., you will play a vital role in providing exceptional care to patients and their families.

About the Role

We are seeking a compassionate and experienced Registered Nurse to join our team as a Healthcare Navigator. In this role, you will be responsible for coordinating patient care, referrals, and admissions to hospice services. You will work closely with discharge planners, physicians, and staff nurses to ensure seamless transitions of care.

Responsibilities
  • Greet patients and families on the phone, answer general information questions, and record patient information as needed.
  • Serve as a resource for information and support to referred patients, families, and referral sources concerning potential hospice care.
  • Follow the appropriate referral intake process when taking referrals.
  • Evaluate information received from referral sources and assess patients for eligibility for hospice services, appropriate levels of care, and request additional information as needed.
  • Ensure timely transfer of all admission information to appropriate team members.
  • Complete the admission process and required documentation in an accurate and timely manner in the electronic health record.
Requirements

To be successful in this role, you will need:

  • A minimum of one year experience as a Registered Nurse (RN) in a hospital, home health agency, hospice, or related program.
  • Licensure as an RN in Arkansas.
  • Valid Arkansas driver's license, car insurance, and a reliable vehicle.
